Completing the online referral
The online referral referral will take around 15 to 20 mins to complete although this does depend on the circumstances of the child/family you are referring. You can't save and return to the form atÌýa later time so make sure you give yourself enough time. You can return to previousÌýsections of the form and each section will be saved as long as you don't close the window. ÌýÌýÌýÌý
You will need to tell us details about the child and their family, including where they live and their date(s) of birth,Ìýso make sure you have these to hand before you start.Ìý
You will also be able to upload documents whichÌýmay help the MASH team with their assessment such as a CAF report or early help assessment, so have these ready to upload too.
Once you submit the online formÌýthe MASH team will receive it immediately. A copy of the formÌýwill be sent back to you (and your organisation if you request it) straightaway by email.
